Job Description:

Position Overview

  • The Director of Case Management at Valley View Medical Center will play a vital role in implementing strategic plans through comprehensive patient assessments, planning, and evaluation.
  • The position emphasizes enhancing the quality of patient management, ensuring continuity of care, and cost-effectiveness while upholding the organization's mission and values.

Essential Responsibilities

  • Develop and implement departmental goals aligned with the organization's vision.
  • Integrate social services and case management functions into patient care and discharge planning processes.
  • Conduct concurrent medical record reviews and ensure compliance with regulatory standards (CMS, Joint Commission).
  • Act as a patient advocate, addressing adverse occurrences and educating staff on resource utilization.
  • Collaborate with physicians and interdisciplinary teams to develop effective care plans.
  • Facilitate patient rounds to optimize care coordination and identify post-hospitalization needs.
  • Oversee patient satisfaction and ensure the quality of care aligns with organizational standards.

Qualifications

Must-Haves:

  • Bachelor's degree in Nursing (BSN) or higher.
  • Current Arizona RN license or Compact State Registered Nurse license.
  • Minimum 5 years of experience as a Director of Case Management in a critical care hospital.
  • Current BLS certification.

Preferred:

  • CCM or ACM Certification.
  • Master’s degree in Nursing or Healthcare Management.
  • Experience in leadership roles within critical care or case management.
