Job Description

Job Title: Licensed Master Social Worker (LMSW)

Duration: 13 weeks

Shift: Monday-Friday, 9:00 AM - 5:00 PM

Location: Bronxville, New York

Pay: $60/hr - $65/hr


Job Description:

We are seeking a dedicated and experienced Licensed Master of Social Work (LMSW) professional to join our team. This position involves working with members and caregivers, assessing their needs, and providing case management services. Responsibilities include traveling within the service area to conduct home visits, facility visits, and engage with social service agencies.


Key Responsibilities:

  • Conduct field visits to members and facilities, and collaborate with clinical providers and community-based organizations.
  • Review and interpret medical records, claims, pharmacy data, lab reports, and prescriptions.
  • Support clients dealing with homelessness, addiction, mental health, entitlements, and housing issues.
  • Assist clients with understanding their rights and responsibilities as tenants.
  • Provide case management services in a hybrid role, working both remotely and on-site.
  • Strong communication with clients, caregivers, and service providers to ensure a coordinated approach to care.


Qualifications:

  • Education: Master’s Degree in Social Work and LMSW licensure (NYSED) required.
  • Skills: 1 year LMSW experience.
  • Job Requirement: Valid Licensed Master of Social Work (LMSW) license issued by NYSED
  • Required : Communication skills, knowledge of Psychosocial Assessment, Masters in Social Work, NYS LMSW license
  • Bilingual (English/Spanish) is preferred.
