Job DescriptionJob Summary The Patient Care Coordinator is responsible for coordinating patient care activities to ensure smooth, efficient, and patient-centered care across various departments. This role serves as a liaison between patients, families, and healthcare providers, supporting patient navigation, addressing concerns, and facilitating communication. The Patient Care Coordinator assists with scheduling, patient education, and care coordination tasks to enhance the patient experience and ensure continuity of care. In addition the role will work as a health coach. Essential Functions Acts as a primary point of contact for patients, coordinating care and supporting communication among patients, families, and healthcare providers. Manages patient scheduling, including appointments, follow-ups, and procedure coordination, to support efficient care delivery. Provides patients and families with information on care plans, treatment expectations, and available resources. Addresses patient questions and concerns promptly, providing guidance or escalating issues to appropriate clinical staff as necessary. Collaborates with healthcare team members to ensure continuity of care and alignment with department protocols. Assists in managing patient records, documentation, and data entry to maintain accurate information for the care team. Participates in quality improvement initiatives to enhance patient satisfaction and care coordination practices. Supports discharge planning and follow-up care coordination as directed by clinical staff. Performs other duties as assigned. Complies with all policies and standards.Qualifications 1-3 years of experience in a healthcare or patient services role required RN License is Required Knowledge, Skills and Abilities Ability to demonstrate effective organizational and time-management skills Demonstrates excellent oral and written communication skills Demonstrates knowledge of basic computer skills Demonstrates knowledge of medical terminology, ICD-10 and CPT coding. Demonstrates knowledge of basic procedures for taking vitals, and obtaining medical information. Ability to problem-solve and multi-task in the work environment
