INTEGRIS Hospital at Home enables our patients to receive acute care at home who would otherwise be admitted into the hospital. Patients consenting to admission into this model of care must meet inpatient admission criteria, live within a 30 mile radius of the admitting hospital and have the clinical and social stability to receive care at home. Common conditions for patients receiving acute care in the home include congestive heart failure, respiratory ailments, diabetes complications, infections like pneumonia, as well as COVID-19.
Patients admitted into this innovative acute care model, receive a technology kit in the home that includes a Wi-Fi enabled phone and remote patient monitoring devices that upload the patient's clinical data into the electronic health record. The INTEGRIS Health @ Home Command Center hospitalists and nurses connect with the patient through telehealth visits scheduled regularly throughout the day coupled with caregiver visits to the home to administer medications, deliver equipment and supplies, and perform clinical procedures.

The Nurse Practitioner provides medical consultation, routine patient care, and follow-up as directed. This position requires population specific competencies. The Nurse Practitioner responsibilities include, but are not limited to, the following:
Initiates and completes a detailed patient assessment.
Initiates the order of laboratory tests, radiological examinations and other routine diagnostic procedures.
Diagnoses the actual or potential health problem or need, based upon an analysis of the data collected, and plans therapeutic intervention.
Orders and initiates appropriate medication, treatment, and consultation based upon an analysis of the data collected.
Orders or performs routine procedures such as injections, immunizations, suturing (not involving tendons, arteries or nerves) wound care, venous and arterial blood draws, and managing simple conditions produced by infection, trauma or other disease processes.
Educates the patient regarding prescribed treatment and medication(s).
The Nurse Practitioner reports to the supervising physician and Operations Manager Practice Management.

Potential for exposure to infections and communicable diseases, blood and body fluids, electrical equipment, chemicals. Must follow standard precautions. Master's degree from an accredited Nurse Practitioner program.
Current Oklahoma Nurse Practitioner license.
Current BLS Certification.
Must be able to communicate effectively in English
