Private Duty LPN – Full-Time Rotating Shifts

Location: Jackson, TN

Overview:
Adoration Home Health & Hospice is seeking two skilled and compassionate Licensed Practical Nurses (LPNs) to provide one-on-one in-home care on a rotating schedule. This position offers a predictable 36/48-hour workweek, providing flexibility and the opportunity to make a meaningful impact in patients’ lives.

Schedule:

Week 1: Monday, Friday, Saturday, and Sunday: 7:00 AM – 7:00 PM (48 hours)
Week 2: Tuesday, Wednesday, and Thursday: 7:00 AM – 7:00 PM (36 hours)

Responsibilities:

Deliver personalized nursing care, including medication administration and vital sign monitoring.
Provide patient and family support while maintaining a safe and clean environment.
Collaborate with interdisciplinary teams to ensure high-quality care.

Certifications Required:

Active LPN license in Tennessee.
Current CPR certification.
Responsibilities:
Perform selected nursing procedures within the scope of LPN practice
Provide patient and family teaching according to plan outlined by the RN
Monitor patient's vital signs and make specific measurements and observations
Monitor and record intake and output. Report significant changes immediately
Observe, assess and record patient's physical and mental condition and reports any changes
Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
Qualifications:
Licensed Practical Nurse / LPN license in good standing
Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
Active CPR certification
Familiarity with electronic health records
Strong problem solving and critical thinking skills
Reliable transportation, valid driver's license and current automobile insurance
