*Job Description:*
* Conduct thorough evaluations of patients’ physical conditions and develop personalized treatment plans to address their needs.
* Provide hands-on therapy, including manual therapy and therapeutic exercises, to improve mobility, strength, and overall function.
* Monitor patient progress, adjust treatment plans as necessary to achieve optimal rehabilitation results.
* Collaborate with a multidisciplinary team to provide comprehensive patient care.
* Educate patients on injury prevention, proper body mechanics, and self-care strategies for long-term recovery.

*Qualifications:*
* Current Physical Therapist (PT) license in the state of practice.
* Experience in Skilled Nursing Facility (SNF) settings preferred, but not required.
* Strong communication and interpersonal skills.
* Ability to work independently and as part of a collaborative team.
* A passion for patient care and improving patient outcomes.
