Territory: Diamondhead and Gulf Coast area

7 days on / 7 days off

ESSENTIAL JOB DUTIES:

Responsible for triaging all after hours patient and family calls, oversight and management of all after hours admissions and visits necessary to ensure all Notre Dame Hospice patients receive required timely care that is in accordance to Notre Dame policies and meeting state and federal guidelines.
Collaboration and coordination of patient care with all necessary members of the interdisciplinary team, including back up staff and the administrator on call.
Provide nursing services with the Plan of Care
Documentation, instruction, education to family/caregivers, Supervise/assist ancillary personnel when necessary.

OTHER QUALIFICATIONS:

Has not been excluded from participating in Medicare and Medicaid programs.
Maintains eligibility to participate in Medicare and Medicaid programs throughout employment.
Capable of providing emergency care as needed for any patient.
Maintains a neat, well groomed appearance at all times. Keeps personal appearance appropriate for patient care.
