*Schedule:* Monday through Friday, Full-Time Days
*Your Role:*
* Ensure activities align with Elara Caring’s vision and leadership.
* Assess physical, psychosocial, and environmental needs of hospice patients and families.
* Assume primary responsibility for caseload, including planning, coordinating, and evaluating treatment plans.
* Conduct initial and follow-up evaluations of patient needs.
* Communicate significant findings to the Clinical Supervisor, physician, and other care team members.
* Revise care plans as needed and counsel patients, families, and facilities.
* Teach hospice philosophy and pain management.
* Supervise home health aides and Licensed Practical/Vocational Nurses.
* Provide support at the time of death and perform bereavement assessments.
* Document and update medication regimens.
* Handle patients' personal property with care.
* Uphold Elara Caring’s philosophy and administrative policies.
* Maintain privacy and confidentiality according to HIPAA.
*Qualifications:*
* Sit, stand, bend, lift and move intermittently and be able to lift 50-100lbs
* Driver's License
* CPR/BLS
* Strong communication skills
* Reliable transportation to perform job duties
* Bachelor’s degree required, with additional one year paid professional experience, or one year as a Registered Nurse
* Preferred to have home care experience
