*Community Options, Inc.* is a national non-profit agency providing services to individuals with disabilities in 12 states.
We are currently seeking an experienced *Occupational Therapist* in *Wayne or Hillsborough, NJ*. The Occupational Therapist provides quality occupational therapy services for patients through functional evaluation and treatments of individuals utilizing a variety of techniques including: activities of daily living, fabrication and application of splints, sensory motor activities. This position requires travel between Hillsborough and Wayne New Jersey Programs. *Responsibilities *
* Screens and identifies individuals who may have problems in occupational performance.
* Evaluates the individual’s performance according to the Uniform Occupational Therapy Checklist.
* Develops individual occupational therapy program indicating short and long-term goals.
* Elects media, methods, environment, and personnel needs to accomplish goals.
* Determines frequency and duration of occupational therapy services.
* Formulates and implements the program according to the program plan.
* Documents therapy services provided, and the client’s occupational performance.
* Discharges clients when they have achieved the established goals and received maximum benefit from the services.
* Periodically and systematically reviews all aspects of the individual occupational therapy program for effectiveness and efficiency.
* Collaborates with client, family, and other professional and community resources.
* May be required to be on-call in cooperation with other management staff
* Additional tasks and responsibilities may be assigned
* Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation

*Minimum Requirements*
* Bachelor’s degree from an accredited school of Occupational Therapy.
* Licensed by the State Board of Examiners in Occupational Therapy.
* Registered with the American Occupational Therapy Association and NBOTA.
* Ability to interact with patients, doctors and other personnel.
* Ability to exercise good judgement and use common sense.
* Excellent interpersonal and communication skills.
* Ability to establish a comfortable and supportive relationship with a person receiving supports.
* Ability to read and write English.
