*Licensed Practical Nurse ( LPN )*

*Responsibilities:*

* Employee aids in the orientation of new employees
* Practices within the LPN scope of practice
* Identifies and reports change in resident status to RN in a timely manner
* Performs and is knowledgeable of vital signs, medication administration, monitoring lab values, care of resident pre/post procedure, geriatric competency, hospice care, pain mgmt., resident safety, restraints, isolation precautions/PPE, admission and discharge of patients/residents
* Employee is able to provide written and verbal educational material to residents/patients/families, and documents education as per policy
* Updates/implements resident plans of care appropriately
* Transcribes and documents physician orders accurately using computer order entry system
* Accurately reconciles medications upon admission and discharge of resident
* Participates in resident feeding and oversees dining areas as assigned

*Requirements:*

* Graduate of an accredited school of practical nursing.
* Licensed as an LPN in New York State
* Licensed as an LPN in New York State
* Basic computer skills
* Previous experience preferred. Knowledge of safety and infection control policies. Knowledge of computer systems and/or prior clerical experience preferred. CPR Certified
