Overview:
- MDS Coordinator (RN) will evaluate and monitor the quality of care provided to long-term care residents.
- Conduct initial interviews and assessments for new residents.
- Collaborate with healthcare team members to support residents in achieving care goals.
- Coordinate and oversee the development and completion of resident assessments (MDS).
- Maintain policies and procedures related to MDS and care planning.
- Assist residents with the care planning portion of their discharge plans.
Qualifications and Requirements:
- Current Connecticut Registered Nurse license required.
- Have experience in conducting assessments (Admission Assessment, Quarterly Assessments, Annual Assessments, Significant Change Assessments, and Discharge Assessments), Care Plan Development and updates, and capturing, coordinating and managing data related to patient assessments (MDS Coordination) in a healthcare setting.
- Experience in developing Care Planning, MDS Coordination (capturing, coordinating and managing data related to patient assessments), Case Mix index, Compliance and documentation, Collaboration, monitoring and reporting. previous or recent Charge Nursing experience highly desired.
- Maintain Case Mix Index (CMI) greater than 1 at 90% or better
- Complete and submit assessments with no errors.
