Job Description

Job Purpose:

A Speech-Language Pathologist (SLP) provides speech or language therapy services in accordance with the patient’s plan of care as directed by their primary care provider.


Supervisor: Clinical Manager


Qualifications:

1. A person who has a master’s or doctoral degree in speech-language pathology and who meets either of the following:

» Is licensed as a SLP by the state in which the individual furnishes such services or

» In the case of an individual who furnishes services in a state that does not need a license

SLPs:

» Has completed 350 clock hours of supervised clinical practicum (or is accumulating such supervised clinical experience).

» Performs at least nine months of supervised full-time speech-language pathology services after obtaining a master’s or doctoral degree in speech-language pathology or a related field.

» Completed a national examination in speech-language pathology approved by the Secretary of Health and Human Services.


Training:

1. At least one year of healthcare experience preferred.

2. Home health care experience preferred.


Responsibilities:

1. Provide comprehensive assessments (recertifications and discharge), including Outcomes and Assessment Information Set (OASIS) items for Medicare and Medicaid patients.

2. Complete patient evaluations (initial, re-evaluations, and discharge) as indicated by the plan of care and/or state and federal requirements.

3. Develop and implement the patient's plan of care in accordance with the patient’s primary care provider.

4. Provides speech or language therapy services in accordance with the plan of care.

5. Evaluate the home environment for safety concerns.

6. Educate the patient and family on speech and or language services.

7. Assign and supervise Home Health Aides as required by Medicare/Medicaid and state regulations, if any.

8. Participates in orientation and training of new personnel.

9. Evaluate and complete competencies for SLPs, as directed by the Clinical Manager.

10. Maintains accurate and timely documentation of care/services provided.

11. Communicate with other team members, as appropriate, regarding the patient’s plan of care,

and updates team members when there are changes.

12. Attends and participates in staff meetings, case conferences, educational in-services, and committee meetings, as appropriate.

13. Report incidents and grievances/complaints (patients and/or personnel) to the Clinical Manager.

14. Report the patient’s progress and effectiveness in meeting goals to the Occupational Therapist.

15. Participate in the patient’s discharge planning.

16. Participates in the agency’s Quality Assessment and Performance Improvement (QAPI) program, as appropriate.

17. Adheres to the agency’s policies and procedures.

18. Maintains confidentiality of all aspects of job performance.

19. Practices proper infection control and utilizes PPE as indicated.

20. Performs treatments or special procedures only as authorized by the agency and deemed competent to perform by the Occupational Therapist.

21. Comply with all applicable federal, state, and local laws and regulations in performing job functions.

22. Performs other related functions as directed.
