Responsibilities and Duties of the Physical Therapist

• Your job is to go to patients’ homes and provide physical therapy. Follow up with notes (electronic) and schedule subsequent visits as needed. Most of our patients have been recently discharged from a hospital and just need some help getting better. You can make a big difference in people's lives.
• This position is responsible for providing services requiring the competencies of a Physical Therapist, to patients in their homes. The Physical Therapist is responsible for evaluation, performing, organizing, and coordinating patient treatment programs in compliance with physician orders and Plan of Treatment. The Physical Therapist organizes and conducts medically prescribed physical therapist programs for patients to restore function and prevent disability following disease, injury, or loss of a body part; helps the patient reach maximum performance within the limits of his/her capabilities. If physical therapy is the primary discipline, then the Physical Therapist is responsible for case coordination and management. The incumbent is responsible for functioning as an integral member of the interdisciplinary team and for completing related duties as required.
• Evaluate the patient’s level of function by applying diagnostic and prognostic procedures and based on physician’s orders and patient assessment, developing, implementing, and revising a plan of care. Services are provided under the plan of care are subject to review and approval by the Director of Patient Care.

Physical Therapist Requirements:

• One to two years (or more) as a licensed Physical Therapist.
• Current California Physical Therapist license in good standing.
• The ideal candidate will have worked in home health industry
• Knowledge of computer charting a plus but we can train.
