Position Summary
Provides pharmaceutical care to patients in their area of specialty according to expert judgment and LRH policies and procedures. The Clinical Pharmacy Specialist works within the scope of pharmacy laws and standards of practice to maximize benefit to the patient while minimizing associated costs. Functions include rounding as a member of the multidisciplinary team, when applicable, and to provide pharmacotherapeutic management for all patients in their respective service lines. He/she coordinates the development and implementation of drug therapy protocols to maximize benefits of drug therapy, and must identify opportunities for and contribute to the improvement of quality, safety, and costs, as well as patient, customer, and employee satisfaction.

Standard Work Duties: Clinical Pharmacy Specialist
- Actively participates in pharmacy team development and accomplishing department goals and objectives.
- Provides comprehensive pharmaceutical care for patients in their area of expertise in a manner consistent with consensus guidelines and evidence-based medicine. Evaluates drug therapy for appropriate indications, dosing, drug interactions, drug allergies, and effectiveness and intervenes when needed. Monitors the patient's pharmacotherapeutic regimen for adverse drug events/reactions and reports these in appropriate institutional system(s) to assist with determining root cause and any potential improvements in the medication use system.
- Actively participates as a member of the multidisciplinary team to provide cost-effective and efficient medication and resource utilization related to area of expertise. Participates in multidisciplinary patient care rounds, when applicable.
- Participates in pharmacotherapy research and/or quality improvement efforts with a focus on improvement in patient outcomes related to area of expertise.
- Coordinates the development, implementation, education, and assessment of evidence-based pharmacotherapy protocols.
- Actively participates on hospital committee(s) related to area of expertise, when applicable. Provides consultation to hospital committees, such as Pharmacy and Therapeutics (P&T), when pharmacotherapy issues related to area of expertise are discussed. Assists with monograph development or formulary reviews when needed.
- Provides education to pharmacists, physicians, nurses, and other healthcare team members regarding current drug-related policies and procedures as well as other pharmacotherapy topics as necessary. Provides education to pharmacy staff in the form of a presentation at least annually, or at the direction of the Pharmacy Clinical Coordinator. Participates in the education of pharmacy residents and students, and overall residency program requirements and activities as outlined in the LRH Pharmacy Residency Program Manual and in accordance with the ASHP Accreditation Standard.
- Identified how drug costs may be minimized through appropriate use of drugs in the specialty area and through implementation of cost-containment measures.
- Organizes job functions and work assignments to be able to effectively complete assignments or projects within established time frames.
- Remains current with all LRH mandatory annual education.

Licenses Essential: Eligible for Florida Pharmacist license.
Licenses Preferred: Active Florida Pharmacist license.
Certifications Preferred: Board Cert; Pharmacotherapy Specialist or Board Certification within area of specialty
Experience Essential: Successful completion of an ASHP-accredited PGY1 residency with at least two years of experience in area of specialty, or equivalent.
Experience Preferred: Successful completion of a PGY2 residency in area of specialty, or have at least three to four years of clinical experience in area of specialty.
