Summary:
Boulder Medical Center (BMC) is currently seeking Medical Assistant and / or LPN candidates to join our team as a Clinical Care Coordinator. This position will work full-time 40 hours per week Monday through Friday.

Experience working in both primary care and specialty care clinics.
Job Duties :
Coordinates healthcare services for patients through assessment of their chronic conditions and/or other health care needs.
Utilizes data provided by Quality Program Manager using both registry functionality withing EHR as well as Payer Reports to identify gaps in care around evidence-based guidelines.
Assists in meeting Value-based care program utilization goals by following up after ER visits and tracking patients discharged to a skilled nursing facilities.
Collaborates with all members of the healthcare team to include the patient, members of the interdisciplinary care team, the patient’s family, and the physicians in the clinic.
Documents care coordination activities in appropriate module database (EPIC, ect.) to ensure proper metric and quality tracking.
Supports refill technicians in addressing prescription refill requests, utilizing standard protocols.
Accurately documents in payer portals to meet quality program requirements.
Collaborates with various healthcare providers across the continuum to ensure that patients are effectively managed and their healthcare needs are met.
Attends and actively participated in quality and program team meetings, webinars and learning collaboratives.

Must possess analytical and computer skills with proficiency in computer and Electronic Health Record systems (EHR) such as MS Office products and NextGen
Must be well-versed in all areas of state and federal regulations regarding HIPAA.
Required Education:
Completion of a Medical Assistant (MA) education program.
Active EMT and / or Surgical Tech certification are acceptable credentials to substitute Medical Assistant (MA) education.
Completion of a bachelor’s degree with an concentration in science / health sciences are acceptable credentials to substitute Medical Assistant (MA) education. Applicant(s) must submit unofficial college transcripts for Medical Assistant (MA) job placement approval.
Previous experience in facilitating quality programs in a healthcare environment, preferred.
Previous Experience:
Must have a minimum of 1-2 years clinical experience.
Must be familiar with standard concepts, practices and procedure within a particular field.
Salary Range:
$18.36 - $23.50 / hour; depending on experience
