Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.
Who We Are We focus on care coordination with patients/families and work closely with members of the interdisciplinary staff to assure that each patient receives the right care in the right setting at the right time. Our care teams are composed of the following:
- Social Workers provide crisis intervention, behavioral health and chemical dependency assessments, screen for abuse and neglect, as well as coordinate post-discharge care. We help coordinate with skilled nursing facilities, home health, hospice, arrange for equipment, and arrange for additional services in the community.
- Utilization Management/Case Management Nurses provide clinical information to our payers and ensure that patients are meeting medical necessity for acute services. They also partner with the Social Workers in coordinating post-discharge care.
- Clinical Documentation Specialists review documentation to ensure that the patient experience is being documented accurately. They partner with the physicians to reflect the appropriate severity of illness and level of service provision.
Job Summary: Role will perform a variety of duties supporting care management initiatives. Provides referral management, utilization review, administrative and clerical support relative to Care Management. Functions as a communication link for the Care Management staff, providers, insurance companies, departments, and other agencies. Working with clinical areas and other key stakeholders, supports data collection processes and makes recommendations for improving data collection methods, accuracy, and reporting. Participates in performance improvement activities in support of departmental and organizational goals.
Primary Duties:
- Coordinates care and discharge activities including post-discharge care facilities, home care and other community resources as directed by the Care Management clinicians.
- .Provides administrative support including word processing, data entry, filing, organizing, and medical record management. Receives direct phone calls and/or voice mail messages and follow up as appropriate.
- Collects, copies, and transmits pertinent clinical and patient demographic information required to complete arrangements for post-discharge care and/or placement.
- Ensures Durable Medical Equipment is arranged as identified by healthcare team.
- Fields, triages, organizes, distributes other information requests.
- .Responds to requests regarding utilization management, including medical review agencies, insurance companies, healthcare team and community resources
- Proactively seek opportunities to improve abstraction and reporting efficiencies and identify non clinical tasks within the scope of the role.
- Work within multiple EMR and support tool systems and maintains confidentiality.
- Practices fiscal stewardship in the role to ensure resources are appropriately utilized.
- Completes special projects as assigned within scope of training and experience with direction and assistance of the Director, Manager or designee
- Performs other duties as assigned.
License, Certification, Education or Experience: REQUIRED for the position:
- High School graduation or equivalent
- 1 years of progressively responsible experience in office or general administrative work
- Current Healthcare Provider BLS certification by date of hire, per department standards
- Excellent customer service and written communication skills
- Excellent oral, visual and aural skills to effectively communicate and process information regarding patients, internal and external customers
- Ability to multi-task and work well under stress
- Possesses excellent organizational skills and attention to detail
- Ability to function independently and to organize work and establish priorities
DESIRED for the position:
- Associate's degree
- 1 year experience in a medical setting
- Successful completion of a secretarial or equivalent program
- Knowledge of community resources and referral process
- Familiarity with medical terminology
