  • BLS certification required
  • MA state licensure required
  • ARRT certification required
  • COVID-19 vaccination required
  • Documentation of MMR vaccination required
  • Documentation of TB test required
  • Health Statement required
  • MGB Skills Checklist required
  • MGB Confirmation of Assignment/POI Form required
    Additional Details
  • No exemptions or waivers for COVID-19 vaccination will be considered
  • Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or proof of immunity required
  • TB skin test screening must be updated if there is a break of more than 6 months
