*Brief Overview*
* Acts as a resident advocate, promoting the welfare and dignity of all residents.
* Ensures that documentation supporting resident care is maintained and updated as required.
* Facilitates the care planning process for residents within the guidelines of state regulations.
* Recognizes that families are an important support for resident care and involves them in the overall plan of care.
* Communicates effectively with family members regarding resident's condition, preferences and needs and addresses concerns in a respectful manner.
* Monitors resident's condition, appropriately responds to changes in condition; participates in the delivery of quality care and services.
* Coordinates resident care and services using sound managerial and clinical judgment, appropriate objectives and high standards of care and customer service.
* Maintains effective communication with staff, peers and administrative personnel and with particular attention focused on neighborhood teams.

* Associate (Preferred)

Experience:
* Nursing: 1 year (Preferred)

License/Certification:
* BLS Certification (Preferred)
* RN (Preferred)
