*Guaranteed Hours*- 40.
*Total Weekly Gross Pay Package*: $2526
*weekly stipends *: $1246/week
* *Requirements:*
2 Years of Experience.
LCSW required.

*Job Description:*
* care management for transition of care planning including home, HH, SNF, IRU, LTAC, hospice, etc.
* Social work psychosocial assessments for safety planning on inpatient psych, assist with difficult placement and bio-psychosocial needs of patients,
* Provide resources and collaboration with community partners to connect patients with needed services
* Experience or 'Must have' skills: Clinical Social Work License in state of Oregon, and Acute hospital experience
