  • American Heart Association BLS certification required
  • License/Certificate Verification attached within 30 days of start

    Job Requirements
  • BLS certification is required for all direct patient care personnel
  • Must be dated within 30 days of start. For any position requiring a license or certification, attach scanned on-line verification page with a machine-generated date indication when the verification was performed within past 30 days
  • Fit Test required
  • NIHSS certification required
  • 9 Panel Drug Screen completed within 30 days of start date, preferred within 14 days of start
  • Trinity Code of Conduct Form signed
  • Standard I9 completed by the agency
  • Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement signed
  • Trinity Integrity and Audit Services Form signed
  • Trinity Specialty Specific Competency Testing within a year
  • Trinity Color Vision test required
  • COVID-19 Vaccine with proof of vaccine Sep 2024 booster, declination or exemption (with agency letterhead)

  • Trinity Education Verification for candidates highest level of academic education
  • Trinity EPLS with scanned report dated within 30 days
  • Trinity GSA/OIG with scanned result print-out dated within 30 days
  • Trinity NSO with scanned report within 30 days of start
  • Trinity System Access Form completed one week before start
  • Trinity Child and Adult Dependent Abuse Training for Iowa
  • Trinity IA Sing Report including Iowa's Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check
