Central North Carolina, Post Acute Care Physician Opportunity
  • Clinical Role:

  • Part-time physician to provide in-person coverage one day per week in skilled nursing facilities /post acute
  • Clinicians trained in or practicing medicine in the following disciplines are encouraged to apply: family, geriatric, primary, internal, adult, acute and post-acute, home health, hospice, palliative and hospital medicine.
  • Compensation is a productivity-based model (Fee for Service). Expected compensation is estimated range of $72k to $102k annually with no cap on productivity income potential
  • Provide oversight and mentorship to our clinicians
  • Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
  • Represent group in engaging, establishing, and maintaining relationships within the community
  • Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees
  • Collaborate with leadership to review care coordination initiatives
  • Develop intervention plans, and share ideas about network management issues
  • Work alongside pharmacists on medication management
  • Complete Medicare required visits
  • Complete acute, follow up, and discharge visits as necessary
  • Implement an individualized plan of care for patients' goals
  • Make recommendations on transitional care, DME, home health services, or outpatient services after a short stay
  • Facilitate and assist leadership in quality improvement initiatives and risk management
  • Timely response in chart completions and pharmacy recommendations
