*About the Role*

We are seeking a dedicated and compassionate Physical Therapist to provide personalized, one-on-one care to patients in their homes throughout the beautiful Lake Winnipesaukee region. You'll enjoy the autonomy to assess, evaluate, and implement treatment plans. *What You'll Do*
* Conduct comprehensive patient evaluations and develop individualized treatment plans.
* Deliver hands-on therapeutic care tailored to each patient's needs.
* Educate patients and families on conditions, treatments, and strategies.
* Collaborate with a multidisciplinary team, including physicians and other clinicians.
* Provide therapeutic interventions, such as exercises, mobility training, and recommendations for adaptive equipment.
* Assess home environments and suggest modifications.
* Maintain accurate and timely documentation using KanTime EMR.
* Actively participate in case conferences and interdisciplinary meetings.

*What You Bring*
* Bachelor's degree or higher in Physical Therapy from an accredited program.
* Active Physical Therapy license in New Hampshire.
* Excellent communication, interpersonal, and problem-solving skills.
* Proficiency in EMR systems and comfort with technology.
* Experience in home health preferred (*we are happy to train passionate clinicians!*)
