Key Responsibilities:

Conduct insightful interviews with residents and families to collect essential social history.
Empower residents and families to actively participate in developing social service programs.
Collaborate to devise innovative solutions for social and practical challenges, including financial assistance and discharge planning.
Work seamlessly with various departments to design impactful social service programs.
Actively participate in facility surveys conducted by authorized government agencies.

Minimum Qualifications:

At least 2 years of experience in a Skilled Nursing Facility (SNF).
Proven ability to collaborate across departments and engage with diverse populations.
