Compass Healthcare Consulting & Placement is conducting a search for an experienced RN Quality Management Coordinator, Registered Nurse, for a Hospice Program based in Manhattan, NY. Position is Part Time, working 3 days per week, in the office. Qualified candidates will have prior Hospice, Quality Management/Performance Improvement, Team Facilitation experience; utilizing Quality tools including application of Statistical Processes, Control Tools, Cops and NYS DOH Regulations.
Responsible for performing chart auditing and focus studies, assisting the Director of Quality Management with the analysis and dissemination of data, participating in Quality Improvement activities & programs in hospice and organization, QAPI Committee and RCAs. Participates in CMS mandated Quality reporting HIS & CAHPS. Maintaining current knowledge of regulatory bodies and standards related to Hospice delivery of services and educates staff regarding quality improvement, regulatory and accrediting body issues.
Responsibilities:
- Performs chart auditing and focus studies and Assists the Director of Quality Management with the analysis and dissemination of data
- Completes required number of audits in a timely manner
- Develops useful audit tools
- Reviews audit findings for accuracy, patterns and trends
- Makes recommendations for performance improvement and shares audit findings with hospice leadership
- Maintains and manages audit result reports in corresponding binders and computer files
- Participates in Quality Improvement activities in hospice and organization
- Assists hospice leadership in identifying areas in need of improvement
- Participates in PI projects by obtaining and managing data
- Documents performance improvement projects and files final report
- Possesses understanding of regulatory bodies and standards related to Hospice delivery of services and educates staff regarding quality improvement, regulatory and accrediting body issues.
- Maintain current regarding regulatory and environmental changes
- Alert hospice leadership to any upcoming changes
- Assist with dissemination of updated regulatory and pertinent information
- Assist hospice leadership with understanding of new regulatory requirements and implementation of new processes
- Participates in and leads QAPI Committee and Root Cause Analysis meetings.
- Completes audits, aggregates data and enters findings on QAPI Dashboard, shares with hospice leadership.
- Reviews results of CAHPS survey, shares results with hospice leadership and makes recommendations for improvement
- Presents Dashboard indicators/results and CAHPS survey results at quarterly QAPI meetings
- Conducts RCA meetings timely with appropriate participants, using RCA tools.
- Documents RCA findings and works with hospice leadership to implement a plan of action.
- Participates in Infection Control, Obtains Infection Control reports from hospice teams following IDTs
- Reviews each reported infection to insure infection is hospice acquired and for patterns and trends
- Prepares the Infection Control Quarterly report and shares with hospice leadership
- Participates in CMS Quality Reporting Program, (HIS) Hospice Item Set and CAHPS.
- Reviews clinical aspects of HIS for accuracy prior to CMS submission.
- Works with Teams to ensure compliance with HIS
- Assists the Interdisciplinary Teams to implement performance improvement projects to increase CAHPS scores
- Participates in Grant Program
- Assist Quality Management Director with implementation of the program
- Provides education and guidance to the Grant Program Customer Service Representatives
- Provides feedback regarding program
Qualifications:
- BSN Bachelor’s in Nursing
- NYS Registered Nurse with active License in good standing
- Experience in Hospice or Quality Management/Performance Improvement, Team Facilitation, application of Statistical Processes Control Tools, Cops and NYS DOH Regulations
- Basic analytical skills required and data manipulation proficiency
- Excellent interpersonal, organizational, written, verbal skills;
- Strong analytical, problem-solving, and data management skills
- Excellent problem-solving skills, with sound judgment
- Ability to prepare and communicate presentations
- Ability to manage many projects at one time with varying deadlines
- Ability to work independently, in groups and with multidisciplinary teams with all levels of the organization
