Position Overview:

Pride Health is seeking an experienced and passionate Registered Dietitian to join our team. This full-time, onsite role is ideal for a professional with expertise in nutrition services, especially in working with frail or elderly populations. The Registered Dietitian will be responsible for conducting nutrition assessments, delivering counseling, overseeing meal delivery, and ensuring the quality and safety of the nutrition program in collaboration with the interdisciplinary team (IDT).

Key Responsibilities:

• Nutrition Assessment : Conduct thorough assessments to determine the nutrition needs, preferences, and goals of participants.
• Care Planning & Nutrition Interventions : Participate in IDT meetings and develop individualized care plans, including special diets and nutrition counseling.
• Menu and Meal Oversight : Oversee the menu planning process and meal delivery, ensuring compliance with safety and regulatory standards.
• Staff Training & Collaboration : Provide training to staff, and collaborate with the IDT to ensure smooth transitions of care across settings.
• Documentation & Compliance : Complete accurate documentation in a timely manner and ensure adherence to privacy and confidentiality regulations.

Qualifications:

• Licensure : Must be a Registered Dietitian (RD) by the Commission on Dietetic Registration (CDR).
• Experience : Minimum of 4 years of experience managing nutrition services in a similar setting, with at least 1 year of experience with frail or elderly populations.
• Skills : Strong organizational, communication, and independent working skills. Ability to prioritize tasks in a fast-paced environment.
• Certifications : BLS (CPR) required.
