Job Details

We are seeking a Physician Assistant to join a interdisciplinary team and deliver patient-centered care to senior participants.

Responsibilities:

• Diagnose and treat acute and chronic conditions, prescribing medications and therapies as appropriate.
• Provide preventative care, health education, and counseling to participants and their families.
• Coordinate referrals and consultations with medical specialists.
• Ensure continuity of care by monitoring participants’ progress and updating care plans as needed.
• Document care provided, ensuring timely and accurate medical records.
• Participate in team meetings to discuss care plans and strategies for patient well-being.
• Serve as a resource for the interdisciplinary team regarding geriatric care.

Qualifications:

• Certified Physician Assistant (PA-C) licensed to practice in California.
• At least 1 year of experience working with an elderly or frail population preferred.
• Strong skills in patient assessment, chronic disease management, and geriatric care.
• CPR/BLS certification required.
• Up-to-date immunizations and medical clearance for communicable diseases.
