Common types of patients & job duties: Discharge to Home, Community, or SNF. Obtaining housing, Shelter Packets for Homelessness, Hemodialysis packers, KCI referrals for wounds vacs, and more. A binder is provided as a resource
Required certs: PRI & SCREEN certifications
