About the Job:

You will provide clinical social work interventions with patients, (neonates, pediatric, adolescents, adults, and geriatrics) and families that address personal and environmental issues to maximize emotional, social, and physical wellbeing and the appropriate and efficient use of health care services across the continuum. You will provide a systematic approach to evaluation of patient care. Coordinate patient care through the continuum thereby facilitating the achievement of optimal quality outcomes in relation to clinical care and cost effectiveness.

What You’ll Do:

  • Provides direct clinical supervision to departmental social workers pursuing licensure, as well as to the social work group.
  • Provides education regarding established workflows, including the EPIC documentation workflow.
  • Assists the Health Care Team in the coordination of care on the unit by serving as a resource to staff
  • Assesses patient/family adaptation to illness/disability and capacity to provide for patient’s care needs via bio psychosocial assessment
  • Collaborates with community social services agencies to enhance discharge planning and continuity of care.

Who You Are:

  • Must demonstrate proficiency in assessing treatment and responses and adapting care to meet the needs of the patient population served; neonatal, pediatric, adolescent, adult, and geriatric
  • Master of Science in Social Work required; NH LICSW Required
  • One (1) to three (3) years of experience in a health care system, preferably in a hospital setting
  • Excellent clinical documentation, and successful usage of EMR systems preferred
