Job Details

Home Health Cases are available in Gloucester County M-F Days

What you will be doing:

• Conducts initial assessments to evaluate patients' physical abilities and develops personalized treatment plans.
• Provides hands-on therapy and guides patients through exercises to improve mobility, strength, and function.
• Educates patients and families on proper techniques for rehabilitation and injury prevention.
• Collaborates with healthcare professionals to coordinate comprehensive care and monitors patients' progress throughout treatment.

Skills you need:

• Licensed as a Physical Therapist in NJ
• Home Health experience preferred
• Oasis experience preferred
