Description
Intake Specialist (IS) (40 hours, Full-time)
BUILD, Inc. (Broader Urban Involvement and Leadership Development) is presently seeking a committed, self-motivated, and innovative individual to join our team. The Intake Specialist (IS) will coordinate clinical intake assessments and facilitate referrals to the appropriate manager housed within the clinical department or to MEL department when other services are needed. The (IS) will receive an initial referral form, make the initial contact, conduct an initial meeting, complete all intake documents along with a narrative and schedule a follow-up with the appropriate manager. The (IS) will be responsible for collecting intake data and furnishing reports for the MEL team, as well as maintaining relationships and meeting regularly with managers from all CCW programs. The (IS) role is a member of the Community Mental Health program within the Clinical and Community Wellness department. The (IS) reports directly to the Manager of Clinical Services (MCS).
Responsibilities are but not limited to:

  • Connects with referred participants in a trauma-informed manner to assess their individual and family needs and complete initial intake paperwork
  • Collaborates with community residents and appropriate stakeholders to facilitate initial intake and assessment
  • Provides referred participants with referrals to community partner organizations when deemed appropriate by CCW managers
  • Maintains current knowledge of CCW/BUILD services, needs, and program capacity
  • Works with community stakeholders to maintain a healthy partnership with consistent and reliable communication
  • Meets weekly with CCW managers to consult on client referrals
  • Submits and maintains initial intake documentation within electronic health record system
  • Provides required reporting needs to CCW managers and MEL team in a timely and consistent manner
  • Operates professionally in the field on a flexible schedule, which may include late evenings and weekends
  • Maintains knowledge current in the field including but not limited to, participation in coursework, seminars, and workshops
  • Attends departmental and agency-wide staff meetings, as scheduled and agency-wide events, as required.
  • Participate in civic engagement activities that advance the mission of BUILD, Inc.
  • Perform other duties as assigned
Requirements
  • Minimum two years of experience working with inner-city youth in a human service setting, trauma-informed care, social service setting, or related field
  • Bachelor's degree required, master's degree preferred
  • Bilingual in English/Spanish preferred
  • Proficiency with technology (Microsoft Office), tech integration into daily program operations and evaluation systems.
  • Maintain respectful and appropriate relationships with program participants, all levels of agency staff and the various publics of the agency, demonstrating sensitivity to all ethnic, racial, and economic backgrounds
  • Self-motivated leader with the ability to communicate in a friendly and informative manner with people from a wide variety of cultural and ethnic backgrounds and lifestyles
  • Must successfully pass an extensive background check

Salary Description
$22.39 - $23.08/hour
