Vivo HealthStaff is recruiting for a Primary Care Physician for a geriatric healthcare organization in San Francisco, California. This is a full-time opportunity with salary and benefits.
The Primary Care Physician conducts in-person assessments and collaborates with the IDT to develop individualized care plans. Under the direction of the Medical Director, this role oversees participant care in nursing homes and hospitals, including admissions, discharges, and the coordination of specialist care. The Primary Care Physician also supports and provides guidance to Nurse Practitioners and other IDT members.
Essential Job Duties:
- Provide assessments, care planning, primary care, and ongoing medical coverage to participants.
- Collaborate with community-based physicians and IDT members in discharge planning from acute or long-term care settings.
- Participate actively in IDT meetings, communicating participant updates, making care planning decisions, and coordinating 24-hour care delivery.
- Oversee care of participants in nursing homes, conducting regular visits as needed.
- Participate in rotating night and weekend call schedules.
Job Requirements: - Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Doctorate degree in medicine.
- Board certification in Internal Medicine, Family Practice, or Geriatrics (preferred).
- Active, unrestricted California medical license.
- Current state CDS certification and DEA registration.
- Eligible for enrollment to provide services to Medicare and Medi-Cal participants.
