Summary:
Monitors, coordinates and evaluates the care and appropriateness of hospitalization; initiates referrals. Participates in development, implementation and analysis of care delivery standards and outcome measures.
Requirements:
Currently licensed as a Registered Nurse in the state of Illinois. BSN preferred. 2-5 years previous Case Management experience preferred.
