JOB PURPOSE:
The Occupational Therapist provides occupational therapy services to patients in the home setting.
Work includes responsibility for the assessment, planning, implementation, and evaluation of patient's
occupational therapy plan of treatment.

QUALIFICATIONS:
- Minimum of one year practicing as an Occupational Therapist.
- Possess and maintain a current license as an Occupational Therapist from the State Board of Occupational Therapy Examiners.
RESPONSIBILITIES: - Conducts a complete physical evaluation in order to determine appropriate treatment in accordance with the functional level of the patient.
- Obtains a complete history through interview of patient/family/others.
- Plans, develops, implements, and evaluates occupational therapy treatment for patients in accordance with the physician's plan of care and the patient's abilities/limitations.
- Completes a plan of care that is thorough with measurable short and long-term goals.
- Teaches home occupational therapy programs to patients and/or their families and assists them in planning/implementing equipment needs for home use.
- Instructs patients in specific skills such as independent daily living skills, grooming and hygiene, sensorimotor, and social interaction.
- Regularly reevaluates the rehabilitation needs of the client and modifies occupational therapy programs based upon those needs.
- Observes signs and symptoms and reports to the physician and/or other appropriate health care professionals as often as needed or as changes in the patient's condition requires
Requirements
SPECIAL REQUIREMENTS - Possess and maintain documentation of freedom of TB in a communicable form.
- Possess and maintain documentation of annual influenza vaccine.
- Mandatory Competencies as defined by the agency Education Plan
- Maintain current CPR certification.
- Maintain current driver's license.
- Maintain current auto insurance of at least $100,000 total liability insurance per person and $200,000 total liability insurance per accident.

PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee
to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling and stooping all day. The employee must frequently lift or move items weighing over 20 pounds. This position requires the ability to transfer, lift, move, and turn patients to and from bed, wheelchair, toilet, tub, shower, and occasionally the floor. Occupational Therapist 2
WORKING CONDITIONS: - Work is performed in the patient's home.
- Exposure Risk to Blood borne Pathogens: Category A - routine exposure to blood and body fluids. Occasional exposure to hazardous materials and infectious diseases.
- Extensive travel is required, sometimes in hazardous weather conditions.
EMERGENCY RESPONSIBILITY: Expected to respond to emergency situations involving the safety of the patient in their homes. This
includes the ability to assist with a possible evacuation of patient, or notify the proper agency/facility in case
of any type of emergency situations such as calling an ambulance or the fire department.
