Responsibilities
  • Performs the initial comprehensive assessment on admission
  • Provides ongoing assessment of needs
  • Monitor effectiveness of services

Qualifications
  • BSN required
  • New York RN license/registration.
  • One year of Hospital Care Management experience
  • Care Management Certification
  • PRI Certification
