We are currently seeking qualified candidates for a full time Long Term Care Liaison - LPN position in the Bingham Farms area.
Job Description:
The Long Term Care Liaison LPN position works closely with the Clinical team, patients, families and nursing facilities.
Qualifications:

• Graduate of an accredited college or university or two years of experience working in hospice and/or home health patient care
• Must be a Licensed Practical Nurse (LPN) in the State of Michigan
• Experience with long term care - required
• Must have good oral communication and writing skills
• Must be able to speak well in public
• Must be able to practice the principles of good public relations
• Must have a current state driver's license
• Must have reliable transportation
• Must have a current auto insurance policy
• Clean background check and drug screen
