Job Title: Physical Therapist
Job Specialty: Therapeutic Treatment
Job Duration: 13 weeks
Shift: Day Shift: 8:00 AM - 4:30 PM
Guaranteed Hours: 40 Hours per week
Experience: Graduate of an accredited Physical Therapy program; GA state licensure required
License: Physical Therapist
Certifications: Basic Life Support (BLS)
Must-Have: Effective communication skills, customer service focus, team orientation, and basic computer skills
Job Description:
- Conduct assessments, plan and organize rehabilitative programs to improve mobility, relieve pain, increase strength, and prevent deformity for patients.
- Work collaboratively with physicians to establish and evaluate treatment programs, plan of care, set goals, and discharge plans.
- Develop plans of care in conjunction with patients and/or family members to ensure realistic discharge plans are set.
- Implement therapy treatments and select appropriate activities to help progress patients towards their goals.
- Revise goals and plans of care based on patient and family input.
- Provide ongoing education and training to patients and families.
- Offer discharge instructions and follow-up referrals to community resources as appropriate.
- Act as a referral source on rehabilitation services and equipment for staff, families, and physicians.
- Maintain accurate documentation of treatment sessions, evaluations, and discharge plans following departmental guidelines.
- Uphold ethical business practices, demonstrate integrity, and maintain patient confidentiality.
- Foster a positive environment for co-workers and patients, striving to provide superior customer service and experience.
- Attend and participate in staff meetings and educational activities necessary for professional development and to maintain licensure.
