We are seeking an experienced Nurse Practitioner with a commitment to high-quality care and patient-centered service. In this part-time position, you'll play a vital role in managing patient health and supporting wellness initiatives in a collaborative clinical environment.
Key Responsibilities

  • Patient Care: Conduct detailed patient evaluations, including comprehensive history-taking and physical examinations. Diagnose, treat, and monitor acute and chronic conditions.
  • Care Planning: Develop, implement, and adjust individualized care plans tailored to patient health goals. Coordinate with other providers and specialists as needed.
  • Chronic Disease Management: Manage conditions such as diabetes, hypertension, and COPD with evidence-based protocols and proactive patient education.
  • Medication Management: Prescribe medications, adjust treatments based on patient progress, and monitor for side effects or interactions.
  • Preventative Care: Perform wellness visits, screenings, and immunizations to promote long-term health and early disease detection.
  • Patient and Family Education: Communicate effectively with patients and families to provide education on disease management, lifestyle modifications, and preventive care.
  • Record Keeping: Maintain accurate and thorough documentation of all care provided using EMR systems while ensuring compliance with billing and coding requirements.
  • Quality Assurance: Participate in quality improvement initiatives to enhance patient care outcomes.
  • Collaboration: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care delivery.
Schedule
  • Preferably three days per week , but open to two days
  • 12 - 15 hours per week
Compensation Options
  • W2 Employment: $60,000 base salary + $35 per visit for billable visits beyond 150 visits/month.
  • 1099 Contract: Per-visit fee structure available for flexible work arrangements.
Qualifications
  • Active, unrestricted Nurse Practitioner license in New York State.
  • Master's or Doctorate degree in Nursing from an accredited program.
  • National certification as a Nurse Practitioner (FNP, AGNP, or similar specialty).
  • Minimum of two years clinical experience as an NP required; long-term care experience strongly preferred.
  • Proven ability to manage chronic illnesses and provide holistic patient care.
  • Strong knowledge of regulatory and compliance guidelines for medical documentation and billing.
  • Exceptional interpersonal, communication, and decision-making skills.
  • Ability to work autonomously in a dynamic healthcare environment.
  • Proficiency with EMR systems and telehealth services.
  • Current BLS certification required.
