Opportunity Highlights

• Schedule: 8am-4:30pm Monday-Friday
• Job Setting: Hospice Home Care
• Type of Cases: Hospice and Palliative care
• Home visits for face to face encounters and symptom management are scheduled as needed, on average 0-2 per day

Minimum Requirements for Consideration

• Board Certified Internal Medicine / Family Medicine / Emergency Medicine
• Hospice and Palliative Medicine or Geriatrics fellowship training is a plus
• Certifications: BLS
• Licensure: Active New York license is required
