Job Description:
- Provide direct nursing care to patients in the home health setting.
- Coordinate with interdisciplinary team members to develop and implement individualized care plans.
- Educate patients and their families on treatments, procedures, and ongoing health care needs.
- Monitor patient progress and adjust care plans as necessary to meet health care goals.
- Maintain accurate and timely documentation of patient care and response to treatment.
- Participate in on-call rotation as required to ensure patient needs are met.
- Ensure compliance with all health care regulations and protocols.
- Communicate effectively with patients, families, and health care team members to provide compassionate and personalized care.
