JOB DESCRIPTION:
The Rheumatology OP Ambulatory Pharmacist Clinician is an advanced practice pharmacist responsible for assuring optimal drug use outcomes in high risk, high cost or complex patient populations. She/He applies specialized knowledge of pharmacotherapy to direct patient care, teaching, and consultation. Provides clinical, pharmacy, and medication expertise in Rheumatology including policy development, pharmacy operations, prior authorization support and medication therapy outcomes.
EDUCATION:
Doctor of Pharmacy and an ASHP accredited residency program or equivalent experience preferred. Licensed pharmacist in the State of Vermont.
EXPERIENCE:
3 years of advanced practice experience in their area of expertise preferred or equivalent experience in ambulatory care or a clinical outpatient setting.
