Job Title: Skilled Nursing RN Charge Nurse

Guaranteed Hours: 48 hours per week
Experience: Prior work experience in a similar or related position preferred
License: Licensure for state of practice required
Certifications: BLS
Must-Have: Graduate of an accredited school of nursing
Job Description:
- Provide care to assigned residents in a caring, safe, and efficient manner.
- Conduct comprehensive nursing assessments for all residents upon admission and on an ongoing basis.
- Demonstrate competence in clinical skills as identified on the Competency Verification and Training Checklist.
- Ensure residents receive ordered medication, treatments, and procedures within defined time frames with proper infection control.
- Be familiar with the therapeutic effects and side effects of all medications administered.
- Obtain necessary equipment/supplies to provide care.
- Follow procedures for pressure ulcers, dehydration, malnutrition, incontinence, pain, and medication administration.
- Accurately complete glucometer testing and control monitoring.
- Implement specific approaches to manage behavior and enhance residents' mood state following the resident care plan.
- Review and update resident care documentation as necessary.
- Utilize electronic medical records (EMR) accurately to document actions and condition changes.
- Record daily skilled nursing notes and document all actions related to admissions, discharges, transfers, or death.
- Complete incident or accident documentation in resident records and necessary forms.
- Participate in quality improvement processes, including audits and corrective procedures.
- Assist in completing the assigned portions of the Minimum Data Set (MDS) for Nursing and sign off on its completion.
- Process physician's orders and notify physicians timely.
- Make rounds with physicians as assigned and attend or obtain resident reports before the shift begins.
- Respond promptly to resident call lights and facilitate resident, family, or significant other education.
- Communicate and follow up on signs or symptoms of resident infections.
