Key Responsibilities Resident Evaluation & Treatment Planning: Assess residents based on their medical diagnoses and create individualized treatment plans to support functional independence. Implementation of Treatment Plans: Execute therapy plans designed to achieve residents' goals, tracking their progress with detailed documentation. Progress Monitoring & Reporting: Prepare recertifications, progress reports, and daily notes to ensure consistent resident care. Confidentiality: Safeguard resident information by adhering to confidentiality protocols. Documentation & Compliance: Maintain accurate records of resident care services and departmental activities. Supervision: Provide clinical supervision to occupational therapy assistants as required. Care Plan Participation: Actively engage in care plan meetings and ensure comprehensive documentation. Departmental Contributions: Assist the Rehab Director with OT-related reports, data tracking, and departmental projects as needed. Qualifications Graduate of an accredited Occupational Therapy Program. Current Occupational Therapist license in New York State.
