JOB SUMMARY:
TheHospital Liaison acts as the primary link between local hospitals and our skilled nursing and long term care facilities. This role involves facilitating patient transitionsfrom hospital settings to our care environment, ensuring smooth communication,and enhancing the relationship between healthcare providers. The idealcandidate will have a strong background in healthcare, excellent interpersonalskills, and a deep understanding of the needs of both hospital patients andnursing home residents.
This position requires frequent travel between nursing home facilities and various hospitals within the Omaha, NE and Lincoln, NE areas. The role may require occasional evening or weekend work to meet with hospital staff or attend community events.
KEY RESPONSIBILITIES:
  • Collaboratewith hospital discharge planners, case managers, and social workers to identifypatients who would benefit from our nursing home services
  • Facilitatethe transfer process, ensuring all necessary medical records and patientinformation are accurately and promptly shared between facilities
  • Serveas the main point of contact for hospital staff regarding patient caretransitions, updates, and concerns
  • Conductregular meetings with hospital staff to discuss potential patient cases, careplans, and follow-up care needs
  • Developand maintain strong, professional relationships with key hospital personnel toensure ongoing cooperation and referral opportunities
  • Representour nursing home at community health events, hospitals, and medical conferencesto promote our services
  • Ensureall patient transfers comply with HIPAA regulations and other healthcare laws
  • Maintaindetailed records of patient interactions, transfers, and outcomes for reportingpurposes
  • Monitorthe quality of the transfer process to enhance patient satisfaction and carecontinuity
  • Addressany issues or feedback from hospitals regarding the care transition process
  • Provide training or informational sessions to hospital staff about our services, capabilities, and care, as needed
REQUIRED QUALIFICATIONS:
  • Minimum of 3 years of experience in a healthcare setting, ideally in patient care coordination or case management
  • Previous experience working in or with skilled nursing or long term care facilties is advantageous
  • Excellent communication and negotiation skills
  • Strong organizational skills with attention to detail
  • Proficiency in electronic health record systems and general computer literacy
  • Ability to work independently and handle sensitive information with discretion
  • Valid RN license or related healthcare certification is highly desirable
  • Valid unrestricted Driver'sLicense, personal transportation may be required
