Position: Social Worker

Type: Direct Hire

Location: Lakewood, CO

Pay: Max 72k


Must Haves:

Masters in Social Work (MSW)

Minimum of 6 months exp. working with elderly

Reliable Transportation, Driver's License, Driver's Insurance

Pluses:

Experience in Epic

Home Health Social Work exp

Hours: 2/5 days M-F 8a-4:30p. 3/5 days are half days-either 8a-12p or 12p-4:30p.

Schedule: 1 full center day at the Lakewood Center when the Social Worker's provider is there, 1 full community day to do home visits, 3 half days to do community work or catch up on charting and patient phone calls.

*The Social Work Supervisor will delegate when the center day will be. It is up to the Social Worker to plan their own schedule to manage their Patient Case Load and prioritize what homes they will visit and when.

Team Makeup: Inter-Disciplinary Team Structure; all team members are experts in areas of their field. There will be an RN, a social worker, PT, OT, and a Registered Dietitian.

The Social Worker will report to the Social Work Supervisor. When fully staffed, there are 8 Social Workers in this center.

Duties & Responsibilities:

The Social Worker will have a caseload of 95 participants. The Social Worker is required to see participants every 6 months OR as needed. The Social Worker can have as few as 10 assessments a month or as much as 20 assessments a month.

These assessments will be in-person psycho- social assessments, monitoring overall well-being (i.e. do the Home Health Patients have all the resources they need).

In order to provide patients with the resources they need, the Social Worker will partner with the client's community resources to provide items such as phones, food, or higher levels of care.

The Social Worker will also go into the client's facility partners to see any of their Patient Case Load that may be in facilities for higher levels of care, and facilitating dispatching of patient case load back to their homes.

Day-to-Day

Center Day:

The Social Worker's day starts with a morning meeting where they go over all pertinent patient updates/needs like acute changes, rehab stay, and discharge planning for respite stays. After the morning meeting, Social Workers can do assessments for their patients in the center if their patients are coming in for appointments or paperwork. After every interaction with their patients or representatives (i.e patient's family, or care givers), the Social Worker will chart in epic.

If a patient comes in and needs to speak with a social worker, they will be tapped in during their center day. During their center days the Social Worker will also communicate with members of their Inter-Disciplinary Team for any patient needs.

Community Day:

Going into the homes of their patients and administering care and monitoring wellbeing.

Half Days:

These days can be spent at home or at the center, catching up on charting or patient phone calls
