Key Responsibilities:
  • Conduct comprehensive assessments for nursing home residents.
  • Develop and implement effective care plans in collaboration with multidisciplinary teams.
  • Monitor patient progress and adjust care plans as needed.
  • Maintain accurate documentation in compliance with regulatory standards.
  • Participate in quality improvement initiatives to enhance clinical services.
Qualifications:
  • Licensure & Certification:
    • Active Nurse Practitioner license and Board Certification in Ohio.
    • Active NPI (National Provider Identifier) and DEA (Drug Enforcement Administration) License.
    • Active Medicare Provider Number in Ohio.
  • Experience:
    • Minimum of 6 months practicing as a Nurse Practitioner.
    • Preferably at least 6 months in a Skilled Nursing Facility or Long-Term Care setting.
  • Additional Requirements:
    • Valid Ohio Driver's License and proof of automobile insurance.
    • Ability to work within the specified hours, Monday to Friday.
