A healthcare organization in New York City is currently seeking a Licensed Master Social Worker (LMSW) to join their growing Inpatient Medicine Department. In this role, the Social Worker (LMSW) will be responsible for identifying psychosocial issues and concerns of patients and their families as well as providing appropriate clinical services to meet their needs.

About the Opportunity:

• Weekend shift only
• Hours: 8am to 6pm
• Setting: Acute Care
• Department: Inpatient Medicine
• EMR: Epic EMRs

Responsibilities:

• Screen charts for historical and clinical information
• Conduct psychosocial evaluations through interviews with patients and/or identified surrogates
• Formulate appropriate social work treatment plans
• Assist in plans for institutional or hospital discharge and other post institutional/hospital care; evaluate alternate care arrangements for patients, where needed
• Confer and consult with professionals in other disciplines to determine and implement a multidisciplinary approach to patient care and to develop treatment plans, evaluate patients’ progress, develop discharge and aftercare plans and recommend alternative action or treatment
• Provide outreach services, make referrals to other governmental, public and/or community agencies
• Prepare patients to accept services, provide direct social work services on high-risk cases and escort patients in connection with community activities and referrals
• Prepare reports and related paperwork; keep appropriate records
• Perform other duties, as needed

Qualifications:

• 1+ year of Social Work experience in a Hospital setting
• Master’s Degree
• NYS Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW)
