Founded in 1970, East Valley Community Health Center is a Federally Qualified Health Center (FQHC) who's services include providing personalized, affordable, high-quality medical, dental, vision and behavioral health care through a community-based network within the East San Gabriel Valley and Pomona Communities. Our staff practices patient-centered care by serving each patient with a personalized care plan that meets their individual needs. Our patients have access to support services that include, nutrition, health education, case management, pharmacy, lab, and x-ray at our health center locations. East Valley serves the health care needs of uninsured and underserved individuals and families throughout our 8 health center locations.

Major Position Responsibilities and Functions:

  • Coordinate access and completion of preventative care (HEDIS) for each patient scheduled, utilizing a huddle list, as well as other lists provided by the QI Team. Determine gaps in care and initiate the implementation of next steps for all standing order items. Coordinate care with the medical provider for additional items that require a medical provider order to be submitted.
  • Responsible for engaging with patients, both in-person and virtually, in a manner that utilizes evidence-based approaches (such as Motivational Interviewing) that promote collaboration between the patient and their medical team and increase the patient's sense of control over their health.
  • Model commitment to continuous quality improvement by engaging in quality improvement initiatives and projects, such as identifying and addressing HEDIS gaps, and by identifying, developing, and testing new practices for improving the outcomes.
  • Identify scheduled high-risk patients prior to their medical visit. Work with Care Team to achieve a successful visit.
  • Identify patient barriers in achieving favorable health outcomes and work with patient and care team to diminish them.
  • Work in collaboration with the referrals department to assist patients with any questions regarding their authorizations.
  • Conduct telephone outreach calls to inform patients about their comprehensive care services and preventative health measures.
  • Provide structured patient education as required.
  • NextGen documentation as required for every encounter and follow-up.
  • Work within Cozeva and maintain documentation.
  • Work with Population Health system and maintain documentation as required.
  • Run Population Health reports on empaneled patients for Care Team meetings.
  • Maintain EHR inbox with all provider/care team tasking requests.
  • Confirm all patient demographics at each visit, or during a patient phone call.
  • Assist patients with patient portal.
  • Actively participates in assigned weekly/monthly care team meetings and daily huddles.
  • Responsible for complying with HIPPA standards and observing strict patient confidentiality.
  • Advise clients and others regarding health care and other facilities available to them; assists patients in utilizing services.
  • Provide clinical staff support within the care team.
  • Other job duties and responsibilities as assigned.
Position Requirements and Qualifications:
  • Medical Assistant Certificate/ Diploma (highly preferred)
  • Basic Life Support (BLS) is required.
  • Ability to multitask and prioritize work in a high-volume environment
  • Must possess high-level computer skills including keyboarding and Microsoft Office and accurate data entry
  • Must be bilingual in English and Spanish with excellent language and communication skills
  • Ability to work efficiently, with minimal supervision
  • Excellent customer service skills, including good phone manners to care for patients and guests on a continual basis
  • Exceptional patient relations and interpersonal skills required
  • Ability to work well with culturally diverse populations

DOE: $21.00 -$24.80
