Description:

• Primary Care/Family Medicine Nurse Practitioner
• Monday - Friday schedule, rotating weekend coverage as needed

Responsibilities:

• Treating patients in the clinic with behavioral health issues and addictions
• Performing H&Ps, mental health assessments and a full spectrum of healthcare services (mainly to adult population)
• Develop and implement patient management plans
• Educate patients on overall wellness and health maintenance
• Maintain a safe and clean work environment

Qualifications:

• At least 1 year of experience preferred
• Active Connecticut License
