About this Opportunity:

- Patients per day: 12 patients/day (36 patient-contact hours/week and 4 hrs. for charting, etc.)

- Hospital Bed Size: 317
- Case/Procedure Experience: BOTOX and ONB preferred
- Hospital Requirements:
- Board Certification: BC or BE in Neurology
- License Required: AZ License
- DEA: Yes
- Certifications Required: BLS or ACLS
- Credentialing Timeframe: 2 weeks
- 1099 contractor
