Position Overview:
Pride Health is seeking a compassionate and skilled Clinical Social Worker to join our Transition of Care Department in Greensboro, NC. This role focuses on working with adult neurology patients, particularly those affected by stroke and seizures, to arrange SNF placement, home health, and durable medical equipment (DME). The position also involves addressing social determinants of health (SDOH), domestic violence, substance abuse, and homelessness. Key Responsibilities:
- Specialize in SNF placement and home health/DME arrangements for adult neurology patients (primarily stroke and seizure cases).
- Address social determinants of health (SDOH), including domestic violence, substance abuse, and homelessness.
- Provide support to patients during transition of care, ensuring proper disposition planning.
- Assist with other patient populations as needed, though the primary focus will remain on adult neurology.
- Float to other Cone Health campuses if required to meet patient needs.
- Maintain accurate patient records and comply with all facility regulations.
- Ensure proper communication with patients and healthcare teams to promote effective care coordination.
Qualifications:
- Experience : Minimum 1 year of clinical social work experience required; first-time travelers are encouraged to apply.
- Licensing : NC state license required at the time of submission.
- Certifications : BLS required.
- EPIC Experience : Required for all applicants.
- Other Requirements : Ability to manage a caseload, strong communication skills, and flexibility to float as needed.
